Non-Court Ordered Subpoenas and Other Legal Documents
You may receive a PCR pursuant to an administrative subpoena or other non-court ordered legal process if you certify in writing that you have attempted to get permission from the patient, or if you have requested a qualified protective order from the court.

Law Enforcement Purposes
Law enforcement personnel may have access to PCRs to the extent the information is relevant and material to a criminal investigation, or to locate a suspect, fugitive, material witness or missing person. Information provided under these circumstances may be limited to the minimum necessary.

These standards apply to oral as well as written disclosures. Therefore, requests to speak to medical care providers concerning medical treatment of a particular patient will be denied unless the above procedures are followed.

Fee
Patients are entitled to one free report. Therefore, the initial request for a patient care report is free. A fee of $10 each will be charged for additional individual patient care reports. The fee is waived for federal and local government agencies and for an order by any court of competent jurisdiction. The timeframe for release of documents is 10-20 days. Release of patient care reports is dependent upon availability.
